FIELD TEST ENTRY Note that the "NAHRA Expiration Date" is now included on Official Field Test Entry Forms Field Test Entry - Editable (pdf) - Two Field Test entry forms that are editable and can be filled out on your computer before printing using the Adobe Acrobat reader. Note that information entered on the first form (except for the test date) is automatically duplicated on the second form. The test date is not duplicated since the tests may be on different days. Field Test Entry - Club Field Test Premium (pdf) - Two Field Test entry forms and a NAHRA membership form for use by clubs to include in their field test premiums. The club name, test location, and date is editable and can be filled out on your computer before printing using the Adobe Acrobat reader. None of the other fields are editable since the form is designed to be mailed with the club's premium. | REPORTS & APPLICATIONS Field Test Number (FTN) Application - INDIVIDUAL (pdf) - If your dog isn't an 'approved breed' and you want to participate in a NAHRA field test, download this Adobe Acrobat form to apply for a field test number. Field Test Application - CLUB (pdf) - Clubs submit this 8 weeks before holding a field test. If your club is planning more than a two-day test, please contact NAHRA Office for a special form. Field Test Report - CLUB (pdf) - This field test report can be filled out using the Adobe Acrobat reader. After the field test, clubs return this form to NAHRA along with a list of qualifying dogs. |
